Must be a Bangladeshi Citizen and should submit copy of National Identity Card.
02 (Two) Passport size photo of the bidder / representative.
Electronic Tin (E-TIN) registration certificate.
Valid Trade License for the fiscal year 2018-19.
Up-to-date BIN (Business Identification Number) certificate.
Registration certificate as manpower supplier issued by respective ministry.
Income Tax Clearance Certificate of Fiscal Year 2017-18 including last 03 (Three) fiscal years (2014-15, 2015-16 & 2016-17).
Bank Statement (Original Computer Sheet) from 01-08-2017 to 31-07-2018.
Financial Solvency certificate stating amount from any scheduled Bank with the bidder holding an account (The statement should be issued after 31-07-2018). Solvency certificate should be based on Bank Statement attached. Certificate Stating “Sound & Solvent” is not acceptable and biddder submitting that type of certificate will be rejected.
Experience certificate of supplying at least 75 (Seventy-five) personnel in any Government Hospital / Autonomous Hospital for at least 01 (One) year. Experience certificate and respective work order must be submitted.
